There is just one box for amendments. Please share:

• Amended Guest Names (please tell us which Guest, eg. Guest 2, Guest 3 etc)
• Amended Meal Preferences (please indicate which Guest)
• Amended/additional dietary requirements (please indicate which Guest)
• Any other information

If you need to let us know that any Guest(s). or yourself, can no longer come – and you definitely no longer require the ticket – please write ‘Guest (x) cannot attend’ in this box. Thank you. We will re-allocate the ticket to someone who otherwise might not have been able to attend.
